This document is about both consent and permission to be able to use certain property to be able, or record and take images at a certain time and date; permission comes from the person who wrote out this letter, and the consent comes from the recepient, the person who agrees to the services by filling out the details at the bottom of the form.
Receipents have to fill out this form in the case that if any of the equipment were to be damaged, or if any of the recepients were to violate any of the rules that the paper(s) require, they can be charged for breaking the rules, damaging equipment, etc. as they have signed the contract, which allows the owner of the equipment/paper to do so.
I would need to use these kind of documents, if I were to grant permission to allow anyone to use any of my images/videos, or anything displayed on my website. Obviously, I have copyright claim to the images/videos, so if anyone were to oppose that and claim my work to be their own, I have full authority to sue/charge them for violating the rules (plagarism) of the paper I would have handed to them.
