This kind of form is a two-way agreement from both the photographer and the performer. If the performer signs his part of the contract, he grants full permission to the photographer to take pictures of him, and the performer can't revoke this or prevent this; once it's signed, it's signed. The photographer signs to agree to this and have the ability to take images and altar them.
In any circumstance that the performer isn't comfortable with having photos of him/her being taken, they don't have to sign the contract, meaning pictures of them won't be taking, and the photographer doesn't have the ability to take the pictures.
I would use something like this in the case I would want someone to feature on my website; I would give them this form, and once they've filled it out, I have the full permission to publish that person on my website.
